DESCRIPTION:Description: Unite Virginia connects health agencies\, social care services\, faith-based organizations\, and school districts through a shared technology platform\, Unite Us. Unite Virginia builds on existing work in Virginia to address social care gaps and health inequities. By integrating health and social care\, Unite Us allows providers to seamlessly send and receive secure referrals\, track their clients’ referral journeys\, close the loop on care\, and report on tangible outcomes across a range of services and sectors. In this session will provide an opportunity to learn about the work of Unite Us in Virginia\, where we’ve been\, where we are\, and where we’re going in 2024 and beyond. In addition\, attendees will view a demonstration of the software\, including:  \n\nSending and Receiving a Referral\nSharing Resources\nAccessing Platform Data\n\nThe Unite Us platform is accessible at no cost for any Virginia community provider.
