The following steps are necessary to apply for a Tier 4 End-Use (Potable) Rainwater Harvesting System<\/p>\n

Complete and Sign the\u00a0Application Form<\/a><\/p>\n

All appllications must be accompanied by private sector evaluations and designs, to include:<\/p>\n

    \n
  1. Design Cover Page*<\/a><\/li>\n
  2. Water storage unit design notes and site drawings (as appropriate)<\/li>\n
  3. Identification of roofing materisl<\/li>\n
  4. System schematic<\/li>\n
  5. Documentation of NSF and ASSE compliance as appropriate<\/li>\n
  6. System sizing based on number of bedrooms or site specific criteria<\/li>\n<\/ol>\n

    *Design Cover Page is example only. Designers have descretion to use their own forms and letterhead provided that they provide the same information.<\/p>\n

    Submit the completed application form, cover page, and all relevant attachment (as PDFs) to rainwaterharvesting@vdh.virginia.gov<\/a>.<\/h2>\n

    Upon completion of the installation of a Tier 4 Rainwater Harvesting System, the following steps are neccesary to obtain an Operating Permit<\/p>\n

      \n
    1. The installer completes the Completion Statement<\/a><\/li>\n
    2. The design professional completes the Inspection & Completion Statement<\/a> (to include as-built diagram; water quality test results per 12VAC5-635-310.B.5 and 12VAC5-635-310.C; confirmation of a continuity of water supply method; and final Operation and Management Manual per 12VAC5-635-350).<\/li>\n<\/ol>\n

      Submit documents needed to obtain an Operating Permit to rainwaterharvesting@vdh.virginia.gov<\/a>.<\/h2>\n

      Once in operation, scheduled inspections of a Tier 4 Rainwater Harvesting System shall be reported to the Health Department having jurisdiction in the location where the system is installed. THIS FORM<\/a> may be used. If the inspector prefers to submit a report under a company letterhead, the submission must contain all the information listed on the Inspection Form.<\/p>\n
